Job Description
Clinical Care:
- Provide expert diagnosis, treatment, and management of acute and chronic conditions affecting elderly patients.
- Lead comprehensive geriatric assessments, including physical, cognitive, and psychosocial evaluations.
- Develop individualized care plans that address medical, functional, and social needs.
- Manage patients with multi-morbidity and frailty in both inpatient and outpatient settings.
Collaboration and Multidisciplinary Care:
- Collaborate with multidisciplinary teams, including nurses, social workers, physical therapists, and pharmacists, to deliver holistic care.
- Participate in case discussions and care coordination meetings to optimize patient outcomes.
Specialized Programs:
- Contribute to or lead specialized programs such as falls prevention, memory clinics, frailty services, or transitional care.
- Provide expertise in palliative care and end-of-life planning when appropriate.
Education and Mentorship:
- Train and mentor medical students, residents, and other healthcare professionals in geriatric principles and practices.
- Participate in continuing medical education (CME) activities to ensure the team stays updated on advancements in geriatric care.
Quality Improvement and Research:
- Lead or contribute to quality improvement initiatives aimed at enhancing geriatric services.
- Engage in or support clinical research focused on aging and elderly care.
Administrative Responsibilities:
- Maintain accurate and thorough medical records in compliance with regulatory standards.
- Participate in hospital or healthcare system committees to improve geriatric care delivery.
