Review patient's drug regimen on a concurrent basis and communicate any suggested changes to the prescribing physician. Verify the physician's original order for appropriateness prior to dispensing
Provide patient education, counselling regarding drug therapy, drug related disease prevention and proper use of medication.
Prevention, detection and resolution of a drug related problem.
Process electronic records of medications dispensed to hospitalized patients. Fill, check and dispense outpatient and inpatient prescriptions issued by physicians
Prepare, package, dispense and deliver unit dose medication in accordance with standard procedures and techniques.
Maintain and control of the floor stocks of drugs kept in the wards and nursing units.
Prepare drug formulations, strength, dosage forms, and packages that are useful in the care of patient and not available commercially but approved by the Pharmacy and Therapeutics committee.
Prepare all sterile products (e.g. Chemotherapy injections, IV admixture, TPN).
Perform routine inspection of all drug storage area within the hospital in order to ensure that no outdated or unusable drugs are present.
Perform any other tasks as assigned by line manager
