Clinical Pharmacist

As a Clinical Pharmacist, you will be involved in reviewing medication orders, conducting patient assessments, and providing evidence-based recommendations to optimize medication therapy. You will educate patients about their medications, monitor therapeutic outcomes, and actively participate in clinical rounds to contribute to patient management plans.

• Evaluate and verify medication orders for safety and effectiveness
• Educate patients and caregivers regarding proper medication use and side effects
• Collaborate with the healthcare team to develop individualized treatment plans
• Perform medication therapy management and monitor patient responses
• Document clinical activities and interventions accurately
• Assist in medication reconciliation to ensure continuity of care
• Stay current on clinical evidence, medications, and therapy guidelines
