Clinical Pharmacist

SearchPlus HR is on the lookout for an experienced Clinical Pharmacist to join our dynamic healthcare team. This role is crucial in delivering high-quality pharmaceutical care and ensuring safe medication practices that enhance patient outcomes.

As a Clinical Pharmacist, you will conduct comprehensive medication reviews, educate patients on their treatment plans, and collaborate with healthcare providers to optimize pharmacotherapy. Your expertise will be critical in monitoring patient responses to medications, managing drug interactions, and providing clinical recommendations based on the latest evidence-based practices.

• Evaluate and verify the appropriateness of medication orders and prescriptions
• Educate patients about medications, including their uses, dosages, side effects, and interactions
• Collaborate across multidisciplinary teams to improve therapeutic outcomes
• Perform medication reconciliation and therapeutic drug monitoring
• Document and communicate any identified drug-related problems and interventions
• Maintain up-to-date knowledge of current medications and treatment protocols
• Engage in continuous professional development and educational activities
