Responsibilities:
- Case Management:
Oversee the case management process, ensuring quality, timely delivery of healthcare services. Collaborate with healthcare professionals to create and implement tailored care plans for patients. - Patient Advocacy:
Act as a liaison between patients, families, and healthcare providers. Advocate for patients' needs and ensure they receive comprehensive and compassionate care. - Coordination of Care:
Facilitate communication among healthcare team members, ensuring smooth coordination of appointments, tests, and consultations. Maintain continuity of care. - Documentation:
Accurately maintain patient records, including assessments, care plans, and other relevant documentation. Ensure compliance with healthcare regulations and organizational policies. - Resource Utilization:
Coordinate healthcare services efficiently, optimizing resources to prevent duplication and ensure cost-effective patient care. Assist in identifying appropriate community resources and support services. - Patient Education:
Provide patients and their families with information about medical conditions, treatment plans, and available resources. Reinforce instructions and recommendations from healthcare providers. - Quality Improvement:
Contribute to ongoing quality improvement initiatives by participating in case reviews and evaluations. Help implement changes to enhance the efficiency of case coordination. - Crisis Intervention:
Respond to and manage crisis situations, coordinating with the healthcare team to resolve challenges affecting patient care.
Qualifications:
- Bachelors degree in Healthcare Administration, Nursing, Social Work, or a related field.
- At least 2 years of experience in case coordination or a similar healthcare role (e.g., Homecare, Hospital, Medical Center, or Clinic).
- Familiarity with healthcare regulations and compliance standards.
- Strong organizational and multitasking skills.
- Excellent communication skills in both Arabic and English.
- Must be immediately available.
