In this important role, you will provide essential medical care to patients within their homes, ensuring they receive top-quality support and treatment tailored to their individual health needs.
Responsibilities:
- Conduct in-home medical assessments to evaluate patient health
- Diagnose and manage both acute and chronic medical conditions
- Develop personalized treatment plans based on individual patient requirements
- Educate patients and their families regarding health maintenance and preventative care
- Administer medications, injections, and other necessary medical procedures
- Document patient interactions accurately to maintain comprehensive medical records
- Coordinate care with other healthcare professionals and specialists as necessary
- Stay current on the latest medical practices and healthcare regulations
