Job Description
Responsibilities:
Oversee and manage patient care plans, ensuring timely and appropriate medical services. Act as a liaison between patients, all BCC stake holders Organize and schedule patient appointment, follow up any referral related to customer journey Ensure adherence to healthcare regulations and standards. Maintain accurate and detailed patient records. Assess patient needs and resources, identifying and addressing gaps. Provide education and resources to patients and their families about care plans. Work with multidisciplinary teams to optimize patient care. Track patient journey
