- Make daily rounds and meet patients upon their admission.
- Interview patients and family members to obtain information about home environment, family relationships, health history and patient personality traits.
- Assist patients and family members to understand, accept medical conditions and follow physicians’ instructions as well as advise them on social problems.
- Evaluate data gathered in terms of the medical treatment plan and social services progress.
- Utilize resources to assist patients in resuming life in the community.
- Arrange the discharge, post-operative care at home and placement of patient in the foster or nursing homes and alteration of anxieties and fears.
- Complete the Social Services Forms about patients’ health progress and keeping these reports in the file.
- Coordinate with Psychiatric and Nursing Departments in the discharge planning of psychiatric patients or long term patients from the Hospital.
- Assist in the presentation of the health educational progress with patients and their family members during the Annual Health week.
- Assist in the discharge planning services which include information referral and community liaises services.
- assist in training students from the university and higher education institute.
- Maintain department’s policies and procedures, goals and objectives, quality management programs, and safety, infection and environmental controls.
- Attend meeting as required and participate on committees as directed.
- Enhance professional growth and development through participation in the hospital educational programs, current interactive in-service meetings, workshops, etc.
- Comply with the standards laid down by the Approved International Accreditation Standards.
- Through the chain of command and/or occurrence variance report, identified any occurrence not consistent with the operation of the hospital or standard of service for patients, employees or visitors and situations that put patients, staff or visitors at risk for injury or other harm.
- Perform other applicable tasks and duties within the realm of his/her knowledge, skills and abilities as required by the Medical Director
