Clinical Pharmacist

SearchPlus HR is thrilled to offer the position of Clinical Pharmacist, where you will be an essential part of our healthcare team dedicated to enhancing patient outcomes through expert medication management.

In this role, you will utilize your clinical expertise to assess medication regimens, provide drug information, and engage in direct patient care activities. Your responsibilities will include collaborating with healthcare providers to optimize medication therapies, conducting medication therapy management, and educating patients on how to properly manage their medications. You will be instrumental in identifying and resolving medication-related issues and promoting safe medication practices.

• Review and interpret medication orders for appropriateness and safety
• Provide consultations to medical staff regarding drug therapy
• Educate patients about their medications, including indications, side effects, and interactions
• Conduct medication reviews and therapeutic drug monitoring
• Participate in multidisciplinary rounds to discuss patient care plans
• Perform medication reconciliation during patient transitions
• Document clinical interventions and maintain comprehensive patient records
• Stay updated on pharmacy regulations, clinical guidelines, and medication therapies
