Clinical Pharmacist

Your primary responsibilities will include evaluating medication regimens for appropriateness, engaging with medical staff to develop optimal treatment plans, and educating patients about their medications. You will have the opportunity to influence patient outcomes positively by participating in multidisciplinary rounds and addressing any medication-related issues.

• Assess medication orders for correctness and potential interactions
• Provide counseling and education to patients regarding their medication therapies
• Engage in collaborative medication management across healthcare teams
• Perform medication therapy management and medication reconciliation
• Monitor patients for therapeutic outcomes and adverse drug events
• Maintain accurate documentation of patient care activities
• Continuously update knowledge on pharmaceutical developments
