Key Responsibilities:
Case Management:
- Oversee and manage the case coordination process, ensuring timely, high-quality healthcare delivery. Collaborate with healthcare professionals to develop and implement customized care plans for patients.
Patient Advocacy:
- Serve as a liaison between patients, families, and healthcare providers, ensuring that patient needs are met and their voices are heard throughout the care journey.
Coordination of Care:
- Facilitate communication and cooperation between healthcare team members to ensure a seamless flow of appointments, tests, and consultations, maintaining continuity in patient care.
Documentation:
- Keep accurate and detailed records of patient information, including assessments, care plans, and other critical documentation. Ensure all documentation is in compliance with healthcare regulations and organizational guidelines.
Resource Management:
- Optimize the use of healthcare services and resources, aiming for cost-effective and efficient care delivery. Assist in identifying and utilizing community resources and support services as needed.
Patient Education:
- Educate patients and their families on medical conditions, treatment plans, and available resources, reinforcing guidance and recommendations provided by healthcare providers.
Quality Improvement:
- Participate in ongoing quality improvement initiatives, contributing to case reviews and helping implement changes that improve the efficiency and effectiveness of care coordination.
Crisis Management:
- Respond to crisis situations in a calm and effective manner, coordinating with the healthcare team to address challenges affecting patient care.
Qualifications:
- Education: A Bachelors degree in Healthcare Administration, Nursing, Social Work, or a related field.
- Experience: Minimum of 2 years of experience in case coordination or a similar healthcare role (e.g., Homecare, Hospitals, Medical Centers, or Clinics).
- Regulatory Knowledge: Familiarity with healthcare regulations and compliance standards.
- Language: Fluency in both Arabic and English (written and spoken).
- Availability: Candidates must be immediately available to join the team.
Required Skills:
- Communication: Strong verbal and written communication skills, with the ability to engage effectively with patients, families, and healthcare professionals.
- Interpersonal Skills: Ability to establish and maintain positive relationships with patients and the healthcare team.
- Organizational Skills: Excellent organizational abilities to handle multiple tasks, maintain detailed documentation, and manage time efficiently.
