Job Description
Plays a pivotal role in the seamless operation for care connect model to deliver best quality of patient care, oversee the business aspects, ensuring well-coordination and effective management with deep commitment to enhancing patient experiences and outcomes.
Key Responsibilities:
Coordinate Patient Care: Oversee and manage patient care plans, ensuring timely and appropriate medical services.
Communication: Act as a liaison between patients, all BCC stake holders
Scheduling: Organize and schedule patient appointment, follow up any referral related to customer journey
Compliance: Ensure adherence to healthcare regulations and standards.
Record Management: Maintain accurate and detailed patient records.
Resource Evaluation: Assess patient needs and resources, identifying and addressing gaps.
Education: Provide education and resources to patients and their families about care plans.
Team Collaboration: Work with multidisciplinary teams to optimize patient care.
Monitoring: Track patient journeys and identify improvements.
