Job Description and Requirements

Duties and Responsibilities:
Ensures safe, appropriate, cost effective medication therapies for patients according to established policies, procedures, and protocols. Accurately collects, organizes and interprets all patient-specific information needed to assure rational, safe therapy for patients in assigned areas. Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose. Identify, resolve and prevent potential medication related problems Provides advice on medication selection, dosages, administration, compatibility, interactions, and toxicity. Assists with pharmacokinetics consult service and drug dosing per organizational protocol. Perform necessary medication reconciliation upon admission, transfer and discharge Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed. Detects and reports suspected adverse drug reactions accurately and in a timely manner. Sustains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection. Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff, as needed. Provides accurate, adequate, and timely drug information to the professional staff. Documents and communicates all clinical activities and interventions accurately and completely.
Education & Professional Experience (essential minimum requirement):
- Graduate of an Accredited School of Pharmacy with Doctor of Pharmacy Degree, Or Master’s Degree in Clinical Pharmacy.
- Must have a licensed of Practice.
- Work Schedule: Weekdays, evenings, nights, weekends, and holidays as necessary.
- Excellent command of oral and written English as well as organizational skills.
- High Computer proficiency (Word, Excel, Access, PowerPoint and Outlook).
- Must be a resident of the South.
