Position Overview:
The candidate will be responsible for contacting patients that are eligible for enrollment and providing necessary wellness coaching to reduce or eliminate high-risk behaviors; encouraging adoption of habits that are conducive to a higher quality of life; helping IQVIA’s patients coordinate medical care and understand any medical conditions they might have; providing primarily face-to-face coaching, counseling, consultation and referral services to manage various chronic health conditions, and lifestyle behavior changes; providing feedback, being involved in problem solving issues related to improved navigation through the healthcare system and coordinating services with the health coaching team.
The candidate will take management responsibility for timely follow-up with patients and for the flawless implementation of all IQVIA’s patient support program.
